An opportunity has arisen for a Telecare Assessor to join a friendly, supportive, busy specialist Telecare Team.
You will be responsible for completing Telecare assessments, plans and reviews to identify creative solutions, including the provision of assistive technologies to enable service uses to remain independent in their own homes. Promote safety, choice, wellbeing and provide support to carers as appropriate. You will also participate in the Telecare duty of prioritising and screening new referrals.
The post holder will need to have excellent customer service, strong organisational, practical problem solving, IT and report writing skills . The ideal candidate will be confident, show caring , empathic and positive approach in working with service users and their carers needs. You will need a high level of personal drive, commitment to develop and maintain effective working relationships with colleagues, social care, health and our partner colleagues to enable best outcomes for service users.
